Sending Glass Slides for Second Opinion or Consultation

Slides sent directly from the original laboratory:

Ask your urologist or the original pathology lab to send the relevant biopsy slide(s) to the address below .  (The address and phone number of the original lab should be present on the pathology report).  It is easier to have slides sent directly from the lab since they will have the nifty plastic slide holders that prevent breakage of the glass slides during transportation. Please send (by e-mail or U.S. Mail) your mailing address so we may send you a written report and allow for more direct communication.

Slides sent directly by you:

Some laboratories will require you to pick up your slides in person, whereupon they will ask you to sign a release. When you obtain the slides ask for a plastic or cardboard crush-resistant slide holder in which to send them. Slides should be sent by Fed-Ex.

If previous biopsies have been performed, it might be helpful to send them as well.   Generally, the more slides we see, the more we feel we have not overlooked anything. E-mailing your PCDigest my allow us to be more helpful, but it is not necessary.

It is not necessary to send the paraffin block at first. If we think it would be helpful, we will ask the laboratory to send it to us. The paraffin block containing the malignant cores are necessary for tumor markers and DNA ploidy studies.

Results:

We can usually e-mail you results the same day that we receive the slides. A written report will take a day or two later. Please include instructions on whether you would like a report faxed and/or e-mailed and which parties are to receive the reports. Please be advised that electronic transmission of information may compromise your privacy. Hard copies will be sent by U.S. Mail to the address(es) that you indicate.

Please indicate if we may post a picture of your cancer in the Prostate Gallery.

Payment:

Our fee for slide review and written report is $225.00 U.S. for a routine consult and $275.00 U.S. for complex consults. Your insurance may cover most if not all of this. If you are a member of a capitated plan, you wont be able to get them to cough up; it might reduce the multi-million dollar salaries of the CEO and his lackeys.

Please print and fill out our insurance form and enclose it with a personal check made payable to "Dr Jonathan Oppenheimer". You will be reimbursed for all portions of your bill covered by your insurance. Recent federal law prohibits private contracting between physicians and Medicare eligible folk. If you qualify for this status, let us know. We accept Medicare reimbursement as payment-in-full for all services covered by that insurance.

We look forward to assisting you and your physician(s).
